We are seeking an experienced and proactive Clinical Supervisor (LPN or RN) to oversee the quality and consistency of care delivered by our non-skilled home care team.
The Clinical Supervisor will provide clinical leadership, caregiver support, quality oversight, and accountability to ensure services are delivered safely, consistently, and in alignment with client care plans, agency policies, and applicable state requirements.
This role is designed for a healthcare professional who is passionate about developing caregivers, improving care delivery processes, and taking ownership of clinical operations. The ideal candidate is a strong leader who is proactive, organized, and able to identify opportunities for improvement before issues arise.
Key ResponsibilitiesClinical Oversight & Quality Management
- Provide clinical oversight and guidance for non-skilled home care services
- Ensure caregivers are following client care plans, service expectations, and agency policies
- Monitor quality of care and identify opportunities for improvement
- Review and manage care-related concerns, incidents, and client service issues
- Provide clinical direction and support to caregivers as needed
- Promote safe, consistent, and compassionate care delivery
Caregiver Leadership & Development
- Oversee CNA and caregiver performance and provide ongoing coaching and support
- Conduct clinical performance evaluations and provide constructive feedback
- Identify caregiver training opportunities and support professional development
- Reinforce accountability while maintaining a positive team culture
- Serve as a resource for caregivers in addressing client care questions and concerns
Documentation & Compliance Oversight
- Review caregiver documentation to ensure accuracy, completeness, and alignment with agency expectations
- Ensure care plans and service documentation are maintained appropriately
- Support compliance with company policies and applicable state requirements
- Identify documentation gaps and provide guidance for improvement

Qualifications
- Active Licensed Practical Nurse (LPN/LVN) or Registered Nurse (RN) license
- Minimum of 3 years of clinical experience
- Home health, home care, or community-based healthcare experience preferred
- Previous experience supervising caregivers, CNAs, or healthcare teams preferred
- Strong understanding of care coordination, documentation standards, and quality assurance
- Excellent communication, leadership, and organizational skills
- Ability to coach, mentor, and hold team members accountable
- Ability to work independently while collaborating with leadership
