About Family Promise of San Gabriel Valley
Family Promise of San Gabriel Valley (FPSGV) is a 501(c)(3) nonprofit organization dedicated to preventing and ending homelessness for families with minor children. We provide emergency motel-based housing, homelessness prevention services, and holistic, family-centered case management.
Position Summary
Family Promise of San Gabriel Valley is seeking a dedicated Case Manager to support the Renter Protection and Homeless Prevention Program. This role provides direct services to individuals and families at imminent risk of homelessness, with a focus on housing stabilization, eviction prevention, and connection to community resources.
This is a grant-funded position and continued employment is dependent on the availability of funding.
Essential Duties and Responsibilities
- Conduct intake assessments to determine eligibility, housing needs, and barriers
- Develop individualized housing stabilization plans with participants
- Provide ongoing case management to households at risk of eviction or homelessness
- Assist participants in accessing rental assistance and other eligible resources
- Educate participants on renter rights and eviction prevention strategies
- Connect participants to community resources including legal aid, benefits, employment, healthcare, and supportive services
- Maintain regular follow-up to support housing stability goals
- Advocate with landlords and service providers as needed
- Complete case notes, documentation, and required data entry in a timely manner
- Ensure compliance with program and grant reporting requirements
- Participate in meetings, case conferencing, and trainings
- Support outreach and program activities as assigned
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or related field preferred (or equivalent experience)
- 2+ years of case management, housing services, or homelessness prevention experience preferred
- Knowledge of eviction prevention, tenant rights, and community resources
- Strong communication, crisis intervention, and organizational skills
- Ability to manage multiple cases and work independently
- Proficiency in basic computer systems and data entry
- Valid California driver’s license and reliable transportation preferred
