Job Title: Physical Therapist (PT) / Occupational Therapist (OT)
Location: Palm Beach County, FL
Position Type: Per Diem – Home Health
We are a rapidly growing homecare agency seeking licensed PTs and OTs to provide skilled therapy services to patients in their homes throughout Palm Beach County.
Responsibilities:
Perform initial evaluations and follow-up visits
Develop and implement individualized treatment plans
Provide skilled therapy to improve mobility, strength, and daily functioning
Educate patients and caregivers
Complete accurate and timely documentation
Communicate with physicians and interdisciplinary team
Requirements:
Active Florida PT or OT license
Home health experience preferred
Current CPR certification
Reliable transportation
Authorized to work in the U.S.
Updated physical and required in-services
Flexible per diem schedule. Pay is based on visit type and patient insurance.
