Duties
- Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or resident’s personal assistance devices
- Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes to healthcare teams promptly
- Administer medications following prescribed care plans while adhering to HIPAA regulations to ensure confidentiality and privacy
- Prepare nutritious meals and snacks aligned with dietary needs and preferences, maintaining cleanliness in kitchen areas
- Support individuals during outings or community activities to promote social engagement and independence
- Maintain detailed documentation of care provided, incident reports, and progress notes in compliance with agency policies
- Engage in light housekeeping duties such as cleaning resident spaces, laundry, and maintaining a safe environment
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
- Knowledge of patient care techniques, including first aid, CPR certification, and basic nursing skills such as vital signs monitoring and medication administration
- Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer’s and memory care), or hospice care is advantageous
- Familiarity with care plans development and implementation for residents with complex needs
- Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
- Strong observational skills for patient monitoring and patient safety assurance
- Compassionate attitude combined with excellent communication skills to build trusting relationships with residents and team members
