This leadership-focused role works closely with residents, families, nursing, and rehabilitation staff to support admissions, care planning, and successful discharges.

Licensed MSW: $83,000-$93,000/year

Licensed BSW: $65,000-$75,000/year

Coordinate admissions and discharge planning

Complete psychosocial assessments, including BIMS and PHQ-9

Support residents and families throughout their stay

Participate in care planning meetings

Collaborate with nursing, therapy, and facility leadership

Maintain documentation and regulatory compliance

Active Social Work license required (LSW, LCSW, or LICSW)

MSW or BSW required

Skilled nursing, rehabilitation, healthcare, or long-term care experience preferred

Knowledge of Medicaid and discharge planning preferred
