Position Summary
The Autism Waiver Case Manager is responsible for the overall administration, coordination, supervision, quality assurance, and regulatory compliance of Reneva Health LLC's Maryland Autism Waiver services. This position will provide program structure, leadership and oversight to ensure the delivery of high-quality services.
The Autism Waiver Case Manager serves as the primary administrator for Respite Care (RC), Family Consultant Services (FC), and Individual Intensive Supportive Services (IISS). Responsibilities include program management, participant service coordination, staff supervision, quality improvement activities, regulatory compliance monitoring, and collaboration with families, service coordinators, healthcare providers, educational systems, and community partners.
Essential Duties and Responsibilities
Program Administration and Oversight
- Direct and oversee the daily operations of all Maryland Autism Waiver services provided by Reneva Health LLC.
- Ensure compliance with Maryland Autism Waiver requirements, Medicaid regulations, agency policies, and applicable federal and state laws.
- Develop, implement, and maintain operational policies, procedures, and service delivery standards.
- Monitor service quality, participant outcomes, operational efficiency, and program performance.
- Participate in strategic planning, program development, and organizational growth initiatives.
- Maintain all required program documentation, records, reports, and operational systems.
- Ensure services are delivered in accordance with participant service plans and approved authorizations.
- Promote person-centered and family-centered practices throughout all services.
Case Management and Participant Services
- Conduct participant assessments, service reviews, and ongoing monitoring activities as required.
- Develop, implement, monitor, and update individualized service plans in collaboration with participants, families, and service coordinators.
- Monitor participant progress toward goals and recommend modifications to services as needed.
- Coordinate communication among participants, families, service coordinators, schools, healthcare providers, and community agencies.
- Assist families in identifying resources and accessing community services and supports.
- Support participants in achieving increased independence, skill development, and community integration.
- Ensure participant rights, dignity, health, safety, and well-being are protected.
- Coordinate crisis intervention activities and ensure appropriate follow-up when necessary.
Respite Care (RC) Oversight
- Oversee the provision of Respite Care services to ensure participant safety and family support.
- Monitor the implementation of individualized respite service plans.
- Ensure staff maintain appropriate supervision and support of participants.
- Review service documentation and ensure compliance with program requirements.
- Support families in accessing and utilizing respite services effectively.
Family Consultant Services (FC) Oversight
- Oversee Family Consultant Services to ensure families receive education, support, advocacy, and resource coordination.
- Monitor family support plans and consultation activities.
- Assist families in identifying appropriate community resources and support systems.
- Ensure family-centered service delivery practices are implemented consistently.
- Support family participation in service planning and decision-making.
Individual Intensive Supportive Services (IISS) Oversight
- Oversee the delivery of individualized intensive support services.
- Ensure implementation of participant goals, behavioral strategies, and skill-building interventions.
- Monitor participant progress in communication, socialization, independent living, and community participation skills.
- Support staff in implementing evidence-informed interventions and person-centered supports.
- Ensure services promote participant independence, self-determination, and community inclusion.
Staff Supervision and Management
- Ensure staff orientation and training is completed based on the treatment plan
- Conduct regular staff supervision meetings and performance evaluations.
- Ensure staff maintain required qualifications, certifications, and competencies.
- Provide coaching, mentoring, and professional development opportunities.
- Monitor staff performance and implement corrective action plans when necessary.
- Ensure adequate staffing levels to meet participant service needs.
Quality Assurance and Regulatory Compliance
- Develop and implement quality assurance and performance improvement activities.
- Conduct audits of participant records, service documentation, and operational systems.
- Monitor compliance with participant rights, confidentiality requirements, and health and safety standards.
- Review and report incidents, complaints, grievances, investigations, and corrective action plans.
- Participate in internal and external audits, inspections, and regulatory reviews.
- Identify trends, implement quality improvement initiatives, and monitor outcomes.
- Ensure agency compliance with all applicable Medicaid and Autism Waiver requirements.
Family and Community Relations
- Develop and maintain collaborative relationships with participants, families, service coordinators, healthcare providers, schools, and community organizations.
- Promote family engagement and participation in service planning and delivery.
- Advocate for participant inclusion, independence, and community integration.
- Represent Reneva Health LLC professionally with community partners and regulatory agencies.
- Support the development of community resources and collaborative partnerships.
Administrative Responsibilities
- Maintain accurate and timely documentation and reports.
- Assist with budgeting, resource allocation, and operational planning activities.
- Participate in agency meetings, trainings, and quality improvement initiatives.
- Maintain confidentiality of participant and agency information.
- Perform additional duties as assigned by the Administrator.
Minimum Qualifications
Education
Bachelor's degree from an accredited college or university in one of the following fields:
- Social Work
- Psychology
- Counseling
- Special Education
- Human Services
- Nursing
- Behavioral Health
- Healthcare Administration
- Related field
A Master's degree is preferred.
Experience
- Minimum of five (5) years of experience working with individuals with Autism Spectrum Disorder, developmental disabilities, behavioral health populations, or related human services populations.
- Experience in program administration, supervision, or case management preferred.
- Experience with Medicaid waiver programs, person-centered planning, and community-based services preferred.
- Experience working with families and interdisciplinary teams preferred.
Knowledge, Skills, and Abilities
- Knowledge of Autism Spectrum Disorder and evidence-informed support practices.
- Knowledge of person-centered and family-centered service delivery models.
- Knowledge of Maryland Autism Waiver requirements and Medicaid regulations.
- Knowledge of participant rights, confidentiality, abuse prevention, and risk management practices.
- Strong leadership, organizational, and decision-making skills.
- Excellent verbal and written communication skills.
- Strong problem-solving and critical-thinking abilities.
- Ability to manage multiple priorities and deadlines effectively.
- Ability to establish positive working relationships with participants, families, staff, and community partners.
- Proficiency with electronic documentation systems and Microsoft Office applications.
Required Certifications and Training
The Autism Waiver Case Manager shall maintain all required certifications and training, including but not limited to:
- Criminal background clearance requirements
- CPR certification
- First Aid certification
- Abuse, neglect, and exploitation prevention training
- Confidentiality and HIPAA training
- Autism-specific training
- Incident reporting training
- Emergency preparedness training
- Person-centered planning training
- Additional training required by the Maryland Autism Waiver Program
