Job Summary
We are seeking a dedicated and energetic Home Health Physical Therapist to join our dynamic healthcare team. In this vital role, you will deliver personalized physiotherapy services directly to patients in their homes, promoting recovery, independence, and improved quality of life. You will assess patient needs, develop tailored care plans, and implement effective treatment strategies across diverse populations, including pediatrics, geriatrics, and individuals recovering from surgery or injury. Your expertise will help bridge the gap between hospital care and community living, ensuring seamless patient care transitions and fostering positive health outcomes.
Responsibilities
- Conduct comprehensive patient assessments to determine functional limitations and develop customized care plans aligned with medical goals.
- Deliver evidence-based physiotherapy interventions such as therapeutic exercises, trigger point therapy, and mobility training to enhance patient recovery.
- Manage discharge planning processes by coordinating with interdisciplinary teams to ensure smooth transitions from inpatient or outpatient settings to home care.
- Provide specialized care for diverse populations including pediatric, geriatric, orthopedic, and neurological patients within the home environment.
- Educate patients and caregivers on physiotherapy techniques, injury prevention strategies, and activity modifications to promote ongoing wellness.
- Document all patient interactions accurately using Electronic Medical Record (EMR) systems while maintaining compliance with healthcare regulations.
- Collaborate with healthcare providers to adjust treatment plans based on patient progress and evolving medical needs, ensuring optimal outcomes.
Qualifications
- Valid licensure as a Physical Therapist with current state registration or certification.
- Proven experience in home health settings with a strong understanding of community-based care delivery.
- Demonstrated knowledge of physiology, anatomy, medical terminology, and clinical practices related to physiotherapy.
- Prior inpatient or hospital experience is preferred; outpatient or clinic experience is also highly valued.
- Familiarity with EMR systems for documentation and patient management.
- Experience in discharge planning, care coordination, and working with multidisciplinary teams.
- Skills in pediatrics, geriatrics, orthopedics, and trigger point therapy are highly desirable.
- Ability to communicate effectively with patients of all ages and backgrounds while providing compassionate care.
