The Case Manager at House of Mercy and Grace is responsible for conducting comprehensive client assessments, developing individualized service and treatment plans, and providing ongoing support to help individuals and families achieve stability, well-being, and self-sufficiency. This role connects clients to essential community resources, public benefits, housing support, and advocacy services while maintaining a compassionate, client-centered approach that aligns with the organization’s mission and values.
Essential Duties and Responsibilities
- Conduct comprehensive assessments to identify client needs, strengths, barriers, and goals.
- Develop and implement individualized treatment and service plans tailored to each client’s circumstances.
- Monitor client progress and regularly evaluate outcomes to ensure goals are being met and interventions remain effective.
- Assist clients with obtaining vital documents, including birth certificates, Social Security cards, and identification records.
- Support clients in applying for Social Security benefits and other public assistance programs.
- Guide clients through applications and processes with the Department of Human Services (DHS).
- Connect eligible veterans with HUD-VASH and other housing assistance programs to promote housing stability and supportive services.
- Advocate for clients and assist them in navigating community systems and available resources.
- Maintain accurate, timely, and confidential client documentation and case notes.
- Collaborate with community partners, service providers, and internal staff to coordinate care and support services.
- Provide crisis intervention, problem-solving support, and referrals as needed.
- Build positive, professional relationships with clients while maintaining appropriate boundaries and ethical standards.

Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred.
- Previous experience in case management, social services, housing support, or behavioral healthpreferred.
- Knowledge of community resources, public assistance programs, and social service systems.
- Experience working with vulnerable populations, including individuals experiencing homelessness, veterans, or low-income families, preferred.
- Strong organizational, documentation, and time-management skills.
- Excellent interpersonal, communication, and advocacy skills.
- Ability to manage multiple cases while maintaining attention to detail and professionalism.
- Ability to maintain confidentiality and handle sensitive information appropriately.
- Proficiency in basic computer systems and case management documentation.
