Responsibilities for the Social Services Assistant position are:
- Supports the Social Services Director with department planning and organization necessary to ensure patient/resident medically related, emotional, and social needs are maintained on an individual basis
- Acts as a liaison and representative of the residents’ social interests.
- Develops social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specifications to be taken
- Will be actively involved in pre-admission and admissions processes
- Must have the ability to listen and act compassionately with residents/family members; must be able to work on one's own and maintain job related jobs
- Documents residents’ behavioral and psycho-social status
- Provides individual and group help for family and residents at all times of adjustment, crisis, or particular need
- Orients resident and family to the facility
- Develops and maintains good working rapport with patients/residents, family members, interdisciplinary team members, service providers, and facility staff to ensure that social services programs can be properly maintained to meet the needs of patients/residents
- Participates in the admission and discharge planning processes, conducts patient/resident assessments, and the care planning process
- Assist in maintaining department reports and ensure records are survey-ready
- Other duties as assigned
Qualifications for the Social Services Assistant position are:
- One year of social work experience in a long-term care facility preferred
- Experience using PointClickCare or other health information management systems preferred
- Excellent interpersonal and listening skills, patient/resident focused, with the proven ability to set priorities and problem solve
