A Human Services Agency is seeking a dedicated and reliable Licensed Practical Nurse (LPN) to join the Nursing team. Reporting to the Registered Nurse and Director of Community Living, the LPN will support individuals with developmental and intellectual disabilities by implementing company policies, assisting in daily operations, and promoting quality community-based care.
Job Role and Responsibilities:
- Provide first aid and basic medical care to individuals and staff.
- Administer, document, and manage medications; request refills as needed.
- Monitor vital signs, assess medical needs, and notify family/providers of illness or injury.
- Observe and document behavior changes, medication effects, and signs of illness; refer for treatment as necessary.
- Communicate medical issues with management staff.
- Track and review annual physicals; update special precaution lists (seizures, allergies, dietary needs, etc.).
- Conduct quarterly nursing reviews and infectious disease training for staff.
- Train staff on healthcare concerns (seizures, diabetes, dysphagia, illness recognition, etc.).
- Assist with direct care (feeding, personal care, toileting).
- Participate in interdisciplinary team meetings and act as a healthcare resource.
- Review incident reports related to illness/injury and document follow-up.
- Complete and maintain Lifetime Medical Histories for individuals.
- Ensure first aid boxes and nursing supplies are stocked and safe; order supplies as needed.
- Report unsafe conditions and perform other duties as assigned.
- Transport clients to doctor appointments and complete training in the group homes.
Qualifications
- Graduate from an accredited school of nursing program
- ASN required; BSN preferred
- Current, unrestricted license to practice as a Licensed Practical Nurse in the state of assignment.
- Two (2) years of experience providing community based supports (residential)
- Valid driver’s license in the state of residence.
