Welcome Homecare is seeking a Full-Time Home Health Registered Nurses to assist our patients in Charlottesville VA area.
· Assess and chart observations of the patient’s condition at each visit.
· Complete evaluation tasks, including reviewing medication and vital signs.
· Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
Requirements:
* Current RN license
* At least one (1) year of recent home health experience
* Able to work in the community, drive and meet patients in their home
* Must be authorized to work in the US
