Job Overview
We are seeking a dedicated and compassionate Physical Therapist Assistant to join our healthcare team. The ideal candidate will work under the supervision of a licensed Physical Therapist to assist in the implementation of patient care plans. This role is essential in providing high-quality rehabilitation services to patients across various settings, including skilled nursing facilities, clinics, and home health environments.
Duties
- Assist Physical Therapists in developing and implementing individualized care plans for patients.
- Provide direct patient care, including therapeutic exercises and modalities.
- Monitor patient progress and report changes to the supervising Physical Therapist.
- Educate patients and their families on treatment plans and home exercise programs.
- Participate in discharge planning to ensure a smooth transition for patients.
- Maintain accurate documentation of patient treatments and progress notes
- Support the physical therapy team in maintaining a clean and organized work environment.
Experience
- Knowledge of human physiology and anatomy is essential for effective patient care.
- Experience working with diverse populations, including geriatrics, pediatrics, and acute care patients.
- Familiarity with discharge planning processes and skilled nursing facility protocols is advantageous.
