Confluence is a short-term residential mental health program providing treatment to young adults with psychiatric, psychological, emotional, and relational challenges. Confluence is a clinically-intensive therapeutic program with a focus on whole person health, including nutrition, physical health and wellness.
Job Description: Program Nurse
As part of the Confluence direct-care team, Program Nurse plays an important role overseeing and maintaining the physical health of participants. This Program Nurse works directly with participants to help maintain wellness and address health concerns as they arise. In collaboration with psychiatric providers and the APRN, the Program Nurse assists in and coordinates all the facets of medical care for participants at the program.
Responsibilities:

Conducts nursing check-ins on participants and supervises wellness plan of care
- Conducts bi-weekly health reviews for participants, maintaining data on biometrics and documents any reported symptoms or health concerns
- Evaluates participant’s clinical health concerns and initiates care in accordance with level of licensure
- Manages daily medication assistance processes ensuring that all participant medications are correctly packaged, available, and appropriately documented and accounted according to regulations and policies and procedures
- Liaises with outside physicians/pharmacies to manage prescriptions and the physical health of program participants
- Trains and assists staff who provide medication assistance
- Liaises with Clinical Team to address concerns regarding participants physical health
- Provides monitoring and education to participants on best practices to improve nutrition, sleep, activity and weight management
- Reinforces an integrated clinical team treatment and wellness plan
- Makes appropriate referrals to outside health professionals for any health conditions
- Assures all assessments, progress and interval notes, biometric data and medication orders are documented in the Electronic Medical Record
Qualifications, Skills and Experience

Registered Nurse with current Vermont RN License
- Experience in a clinical nursing position in an acute, ambulatory, mental health, school or long term-care setting
- Demonstrated competence in nursing assessment, medication administration, and patient education, physical examination and diagnosis and management of common health concerns of adults
- Psychological/psychiatric training or experience a strongly preferred
- Interest in working with young adult populations
- Effective interpersonal skills and ability to work independently
