The person who fills this position is responsible for implementing the Community Living Options Information Process (CLOIP) for adult residents of State Supported Living Centers, primarily Mexia State Supported Living Center. This position prefers two (2) years of experience assisting consumers with disabilities and their families in identifying specific needs and developing plans to meet those needs. Experience working with residential staff focusing on the identification of individual needs, and planning and coordinating service provision to meet those needs is preferred. Exceptional communication and teaching skills, the ability to build working relationships, to conduct effective interviews, and to gather/organize information from observations and interviews are essential. Writing skills are critical. Knowledge of community resources that support people with Developmental Disabilities to participate in community life is essential. Computer literacy and data entry skills are required
EXAMPLES OF WORK PERFORMED:
- Provides training to consumers, parents/guardians, family members, residential staff, and other professional and para-professional staff relating to services and support available to people who have Developmental Disabilities, autism, medical needs and other social, emotional and developmental challenges.
- Interviews with consumers, parents/guardians, family members, residential staff, and other professional and para-professional staff in order to gather information that is used to build a relationship with each consumer, as well as to guide you through the presentation of Community Living Options.
- Completes the CLOIP document in the format required by the Health and Human Services Commission (HHSC).
- Completes SmartCare data-entry related to the CLOIP process with individual residents and assists in maintaining Consumer Records for these same individuals.
- Serves as liaison between the LA and Mexia State Supported Living Center, to include participation in quarterly service collaboration meetings, participation in annual staffing for State Supported Living Center residents and to facilitate enrollment into HCS.
- Provides education and training for other professionals, consumers and their families, regarding service options in community settings and enrollment policies and procedures.
Experience:
Valid Texas driver's license and proof of auto liability insurance.
Education:
High School graduate plus two (2) years’ experience with IDD qualifications or associate degree in a Human Service Field or bachelor's degree in any field.
Knowledge, Skills, and Abilities
- Excellent written communication and computer keyboarding for inputting clinical documentation
- Must have computer skill knowledge of Work, Excel, Outlook and have organizational/filing skills.
- Excellent verbal and telephone communication with consumers, staff, and community providers
- Time management required to meet service goals
- Local travel 30%
- Stooping, bending, sitting, standing, and keyboarding for prolonged periods, lifting up to 15lbs.
Education and Experience
- HS/GED
- 2 years of experience in case management and/or IDD preferred
Hours: M-F 8AM-5PM
