We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surrounding areas. This position offers a rewarding opportunity to make a significant impact in the lives of those we serve.
Key Responsibilities:

Conduct initial and ongoing assessments of patients’ needs to develop comprehensive care plans.

Provide emotional support and education to patients and families about end-of-life care and self-care techniques.

Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress.

Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care.

Maintain accurate clinical documentation and participate in performance improvement initiatives.
Requirements

Current Michigan Registered Nurse (RN) License in good standing.

Minimum of 1 year experience in hospice or palliative care is a plus.

Ability to work collaboratively with an interdisciplinary care team.

Empathetic and compassionate approach with patients and families.

Current driver's license.

Eligible to work in the United States.

Able to travel across Flint, Lapeer, and surrounding areas for home visits.

Willingness to participate in weekend on-call duties on a rotating basis.

Reliable personal vehicle for home visits across the surrounding areas.
