Responsibilities:
- Conduct clinical assessments (ANSAs, CANS) and develop individualized treatment plans for clients with mental health needs
- Collaborate with interdisciplinary team members to ensure coordinated care and effective treatment outcomes
- Monitor and evaluate the progress of clients, adjusting treatment plans as necessary
-Attend PSR team meetings to establish clear expectations, provide educational updates, coaching, policy review, share critical announcements and reminders
Ensure you are available and responsive to assist employees (case managers) with escalated issues and provide support and partnership for more complex cases
Organizational Accountabilities

- Ensure compliance with all home visit protocols inclusive of obtaining patient consents, photo, and completion of progress notes
Attend complex case review meetings and provide requested data to support ensure successful case review as appropriate
- Participate in all PSR team meetings and training
- Responsible for maintaining all required certifications and licensure
- Additional responsibilities may be assigned to support patient or business needs
Skills:
- Valid LPC (Licensed Professional Counselor) or LCSW (Licensed Clinical Social Worker) or LMFT with clinical experience in a relevant field
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with a diverse team
- Strong problem-solving skills and the ability to make sound clinical judgments
- Ability to provide clinical counseling and behavior management support to clients
