Responsibilities
- Provide skilled nursing care in patients’ homes
- Complete start of cares, revisits, and care coordination
- Educate patients and families on disease processes, medication, IV infusions, wound care, foley care and other nursing scope procedures.
- Identify early changes and intervene proactively
- Communicate with providers and interdisciplinary team members
- Document clearly and thoughtfully (narrative matters here)
What We’re Looking For
- Active RN license (Arizona)
- Home health experience preferred (but not required for the right nurse)
- Strong assessment and communication skills
- Self-motivated and organized
- Comfortable working independently
- Values autonomy, accountability, and heart-centered care
