We are actively seeking a PRN Occupational Therapist (OT) to support our growing patient base. This is a per diem role designed for flexibility—perfect for clinicians who want to pick up visits based on their availability without being tied to a full-time schedule.

Responsibilities:

Perform initial and ongoing patient evaluations
-
Develop and implement individualized plans of care
-
Provide therapy focused on activities of daily living (ADLs) and functional independence
-
Complete OASIS assessments as required
-
Educate patients and caregivers on treatment plans

Maintain timely and accurate documentation
-
Communicate patient status and changes with the care team
Qualifications:
-
Active Occupational Therapist license in the state of employment
-
Minimum of one year clinical experience (home health preferred)
-
Strong assessment and documentation skills
-
Reliable transportation and valid driver’s license

Ability to work independently in the field
Compensation:
-
$50 – $80 per hour equivalent, based on visit type and productivity as well as location

Mileage reimbursement available
