Schedule: Monday–Friday | 8:30 AM – 5:00 PM

Physical Therapist – Home Health | Princeton South, NJ
We are seeking a compassionate and experienced Physical Therapist to join a dedicated Home Health team serving the Princeton South, NJ area. In this role, you will provide skilled physical therapy services to patients in their homes, helping them regain mobility, improve functional independence, and enhance their overall quality of life.
This position is ideal for a licensed Physical Therapist who enjoys working independently while collaborating with an interdisciplinary healthcare team to deliver personalized patient care.
Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide therapeutic exercises, gait training, balance training, and functional mobility interventions.
- Assess strength, range of motion, balance, coordination, and functional abilities.
- Educate patients and caregivers on treatment plans, home exercise programs, and injury prevention.
- Monitor patient progress and modify treatment plans as needed to achieve established goals.
- Document evaluations, treatment sessions, progress notes, and discharge summaries accurately in the electronic medical record.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate patient care.
- Ensure compliance with all home health regulations, documentation standards, and patient safety protocols.
- Travel to patients' homes within the assigned service area.
Qualifications
- Current Physical Therapist (PT) license required.
- Previous Home Health experience preferred.
- Strong knowledge of orthopedic, neurological, and geriatric rehabilitation.
- Excellent patient assessment and clinical decision-making skills.
- Outstanding communication and interpersonal skills.
- Ability to work independently and effectively manage a daily patient schedule.
- Basic computer skills and experience documenting in electronic medical records (EMR).
