Help patients regain mobility, confidence, and independence while serving two close-knit communities in southeastern Minnesota. Blue Stone Therapy is seeking a Physical Therapist to join our rehabilitation team serving Spring Valley Care Center and Grand Meadow Senior Living.

What You’ll Do
-
Evaluate patients and develop individualized plans of care
-
Deliver evidence-based physical therapy focused on improving mobility, balance, strength, safety, and functional independence

Provide rehabilitation services at Spring Valley Care Center and Grand Meadow Senior Living

Collaborate with physicians, nursing staff, families, and interdisciplinary team members to achieve the best patient outcomes

Supervise Physical Therapy Assistants in accordance with state licensure requirements
-
Complete timely, accurate documentation supporting quality patient care and regulatory compliance
Requirements:
Minimum Qualifications
-
Graduate of an accredited Physical Therapy program
-
Current Physical Therapist license (or eligibility for licensure) in Minnesota
