Certified Occupational Therapy Assistant
Location: Petaluma , CA
Schedule: Full-time or Part-time options available

We’re seeking a Therapist to join our in house therapy team in Petaluma, CA. We are a 5 star, 90 bed skilled nursing facility. Our skilled mix is usually between about 25-35. We are in-house with our rehab team with longevity of our members. Full time or Part time.Enjoy strong management support, in house benefits and perks as well as the chance to focus on patient care.

Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy interventions to patients recovering from illness, surgery, or injury
- Work with patients who have medically complex needs in a post-acute/SNF environment
- Collaborate closely with PTAs, OTs, SLPs, nursing staff, and families to ensure comprehensive care
- Document progress accurately and timely to support quality care and compliance
Qualifications
- Licensed OTA in the state of California or ability to obtain license
- Experience in post-acute or SNF setting preferred, but new grads are encouraged to apply
- Strong communication, collaboration, and clinical skills
- Passion for patient-centered rehabilitation

Pay: $45.00 - $55.00 per hour
