Registered Nurse - Home Health
Responsibilities:
- Provide specialized nursing care to patients with special needs, including but not limited to ostomy care and dementia care.
- Conduct comprehensive assessments of patients' health status and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report any significant changes to the healthcare team.
- Educate patients and their families on managing their conditions and promoting overall wellness.
- Collaborate with interdisciplinary healthcare professionals to coordinate patient care.
- Document patient information accurately and timely using electronic medical record systems like Kinnser.
Skills:
- Valid Registered Nurse (RN) license in the state of practice.
- Minimum of 1 years of nursing experience, preferably in home health or a related field.
- Strong clinical skills and knowledge in specialized areas such as ostomy care and dementia care.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Ability to prioritize tasks and manage time effectively in a fast-paced environment.
- Proficiency in using electronic medical record systems like Athenahealth.
