Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary.
- Observe and monitor patient conditions, including vital signs and overall health status, documenting changes accurately in Electronic Health Record (EHR) or Medical Record systems.
- Administer medications following medical guidelines and ensure proper medication documentation in compliance with HIPAA regulations.
- Support clients with memory care needs, including dementia and Alzheimer’s care, by implementing specialized techniques to promote safety and comfort.
- Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation.
- Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and organizing living spaces.
- Communicate effectively with clients, families, and healthcare teams in both English and Spanish to ensure comprehensive care delivery.
Skills
- Proven experience working with seniors or individuals with developmental disabilities in assisted living or home health settings.
- Strong knowledge of medical terminology, CPR certification, catheter care, hospice care, and dementia/memory care techniques.
- Familiarity with EMR/EHR systems for accurate medical documentation and patient records management.
- Ability to perform patient observation skills diligently while maintaining compassion and patience.
- Experience assisting with ADLs, medication administration, vital signs measurement, and using mobility aids like Hoyer lifts.
- Excellent communication skills in both Spanish and English to facilitate clear interactions with clients and team members.
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information.
