Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or resident’s <PERSON>
- Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare teams
- Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance
- Prepare nutritious meals tailored to individual dietary needs, including cooking and meal planning for residents in assisted living or home care settings
- Maintain cleanliness of living spaces by performing light cleaning duties to promote hygiene and comfort
- Document care activities meticulously, including patient observations, progress notes, and adherence to care plans
- Engage residents in social activities and provide companionship to foster mental stimulation and emotional well-being
Skills
- Proven experience in home & community care, assisted living, nursing homes, or group homes with a focus on senior or disability care
- Strong knowledge of patient observation techniques and vital signs monitoring
- Certified in CPR and First Aid; familiarity with dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism is highly valued
- Ability to perform heavy lifting safely using equipment like Hoyer lifts; physically capable of supporting residents’ mobility needs
- Excellent communication skills for working collaboratively with healthcare professionals, families, and residents
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information
- Experience with medication administration, care plans development, and patient monitoring in various healthcare environments
- Compassionate attitude combined with patience and adaptability to meet diverse resident needs
