Registered Nurse (RN) — Non-Medical Homecare Agency, Inc.
Position summary
Registered Nurse to support a non-medical home care agency by performing nursing assessments, care plan oversight, clinical guidance, caregiver training, quality assurance, and coordination with clients, families, and community providers. This role focuses on ensuring safe, person-centered non-medical care (personal care, activities of daily living, safety) delivered by homecare aides while addressing clinical needs, risk mitigation, and regulatory compliance.
Key responsibilities
- Conduct initial nursing assessments of new clients in the home and periodic reassessments to evaluate health status, functional ability, safety risks, medication use, and care needs.
- Develop, review, and update individualized care plans based on assessment findings, client goals, and interdisciplinary input.
- Provide clinical oversight and supervision of non-medical caregivers; clarify care tasks, set priorities, and ensure adherence to care plans.
- Train and educate caregivers and family members on personal care techniques, safe transfers, infection control, documentation, observation/reporting of changes, and emergency procedures.
- Monitor client status, identify clinical changes or potential risks, and coordinate timely referrals to physicians, home health agencies, or emergency services as needed.
- skin integrity assessments, simple clinical observations (vital signs if within scope), and document findings per agency policy.
- Review and advise on medication management issues (adherence, administration by caregivers if permitted by policy), and communicate concerns to prescribing providers.
- Conduct regular quality assurance audits, chart reviews, and care visits to ensure compliance with agency standards and regulatory requirements.
- Maintain accurate clinical records and complete required reporting, incident documentation, and regulatory documentation in a timely manner.
- Participate in case conferences, interdisciplinary team meetings, client transitions, and discharge planning.
- Support infection prevention, safety protocols, and emergency preparedness for clients and staff.
- Serve as a clinical resource for agency leadership, providing input on policies, training curricula, and improvement initiatives.
Required qualifications
- Current, unrestricted RN license in [State].
- Active clinical experience (minimum 2 years preferred) in home health, long-term care, hospice, community nursing, or acute care with demonstrated assessment skills.
- CPR/BLS certification.
- Valid driver’s license, reliable transportation, and ability to travel to client homes as required.
- Ability to pass background checks and drug screening per agency policy.
Preferred qualifications
- Experience supervising or training non-medical caregivers.
- Prior experience with non-medical homecare agencies.
- Certifications such as OASIS, wound care (optional), gerontology, or case management are a plus.
