Job Summary
We are seeking a compassionate and dedicated Home Health Aide (HHA) Certified to provide exceptional in-home care to clients across diverse needs. As an HHA, you will play a vital role in supporting individuals with their daily activities, ensuring their safety, comfort, and well-being. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; accurately document medical records and updates in Electronic Health Record (EHR) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care strategies and behavioral management techniques
- Assist with medical procedures such as catheter care, wound dressing changes, and hospice support as directed by healthcare professionals
- Prepare nutritious meals tailored to dietary needs and assist with feeding when required; perform light housekeeping tasks including cleaning and laundry to maintain a safe environment
- Utilize medical terminology knowledge to communicate effectively with healthcare teams and document patient progress clearly and thoroughly
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated experience caring for individuals with developmental disabilities or chronic conditions such as dementia or Alzheimer’s disease
- Familiarity with EMR (Electronic Medical Records) or EHR systems for documentation purposes
- Long-term care experience involving patient observation, medication administration, and vital signs monitoring
- Hands-on experience using mobility aids like Hoyer lifts or other assistive devices in patient transfers
- Knowledge of hospice care principles and working compassionately with patients nearing end-of-life stages
- Proven ability to work independently while maintaining excellent communication skills within a caregiving team
