Job Description:
Job Summary: The Licensed Clinical Social Worker-Palliative Care is responsible for providing psychosocial assessments, and addressing psychosocial, emotional, and spiritual needs for assigned adult/geriatric patients and families facing irreversible serious illness. Provides crisis intervention, emotional support, counseling, education, and advocates for patients/families. Collaborates with the interdisciplinary team in the development, implementation, and evaluation of the treatment plan.
Essential Job Functions & Accountabilities:
Conducts psychosocial assessments for patients and families to identify opportunities for intervention.
Develops, implements, and evaluates patient treatment and care plans.
Collaborates with patients, families, clinical staff, and the palliative care team to identify short and long-term goals of care.
Participates in Interdisciplinary Team huddles and collaborates with other departments.
Demonstrates advanced knowledge of various counseling modalities and how to work with the adult population.
Provides counseling for grief/loss and identifies families at risk for complicated grief.
Provides community resources and support relevant to the patient’s serious illness based on need.
Provides education to patients and families regarding palliative care, advance care planning, and other needed services.
Navigates challenging conversations about end-of-life decision-making while maintaining an inclusive framework that respects and identifies multicultural concerns, ethical issues, potential conflicts of interest, and spiritual or religious concerns.
Collaborates with the clinical staff and the patient/family and provides interventions designed to minimize stress to expedite recovery.
Maintains clinical competence for adult, geriatric, and palliative Social Work services and exhibits professional growth and development.
Participates in quality improvement initiatives.
Maintains electronic medical records with accuracy and in compliance with JPS standards.

Qualifications:
Required Qualifications:
- Master’s Degree in Social Work from an accredited university.
- Licensed Clinical Social Worker by the Executive Behavioral Health Executive Council (formerly known as Texas State Board of Social Work Examiners).
- Current JPS recognized Cardiopulmonary Resuscitation (CPR) Certification and must be maintained throughout employment; may be obtained within 120 days from the date of hire.
Preferred Qualifications:
- Experience working with life-threatening conditions, chronic disease, and end-of-life care.
- One (1) year experience in palliative care and/or hospice care.
- Certification: Advanced Palliative & Hospice Social Work Certification (ASHSW-C).
