Job Description:
We are looking for a dedicated Medical Social Worker (MSW) to assess and address the psychosocial needs of our patients. The MSW will provide counseling, resource coordination, and advocacy to enhance patient well-being.
Responsibilities:
- Assess patients’ psychosocial needs and develop care plans.
- Provide counseling and emotional support to patients and families.
- Connect patients with community resources and services.
- Advocate for patients’ needs within the healthcare system.
- Collaborate with healthcare teams to address social determinants of health.
- Maintain accurate documentation of assessments and interventions.
Qualifications:
- Master’s degree in Social Work and valid Texas MSW license.
- Minimum 1 year of experience, preferably in home health or medical social work.
- Strong knowledge of community resources and social work practices.
- Excellent interpersonal and crisis intervention skills.
- Reliable transportation and a valid driver’s license.
- Ability to work independently and manage complex cases.
