- Driver’s License or Photo ID
- SS Card
- Professional License (RN), (LPN), (CNA) or Certified Home Health Aide Certificate (Must be FDoE approved)
- Level 2 background screening with ACHA fingerprints -https://info.flclearinghouse.com/
- Free of Communicable Disease Statement – Dated/Signed within last 6 months
- CPR card (Only American Heart Association, American Red Cross, OR American Safety and Health Institute
- HIV Certificate 1 hr
- Alzheimer’s/Dementia 2 or more hr approved by DOEA only
- FREE Initial 1-hour DoEA ALZHEIMER’S TRAININGhttps://elderaffairs.org/adrd-training/one-hour-training/
- Must watch video on website
- Once the video is complete, in approximately 30 seconds, a form will pop up. Complete form, confirm correct email, then submit.
- An email confirmation will be provided shortly. Typically, the Certificate of Completion is provided within 24 hours.
- Assistance with Self Medications or Self Administration of Medications 2 hrs
- Auto registration and auto insurance if providing transportation
- Previous Employment/ Contract history for the last five years
- Professional Liability Insurance (Strongly Suggested)
- Occupational Accident Insurance (Strongly Suggested)

Pay: $20.00 - $23.00 per hour
