Summary
We're seeking an experienced and compassionate Program Supervisor to lead our Case Management team. This position oversees program operations, supervises case management staff, and ensures compliance with federal, state, and organizational requirements while delivering high-quality, person-centered services. The ideal candidate has experience working with individuals with intellectual and developmental disabilities (IDD), along with strong leadership and case management skills.

This position is 37.5 hours per week with a pay rate of $28/hour. Westbay offers a generous benefits package including paid vacation, 10 sick days, 2 personal days, 14 paid holidays and a paid day off on your birthday. Qualifications
- Bachelor’s degree in social work (B.S.W) or related field required
- Minimum of two (2) years of professional experience in aging services, intellectual and developmental disability (IDD) services, or a related human services field required
- One year experience managing and supervising staff, required
- Experience in budgeting and fiscal responsibility, preferred
- Pass a BCI clearance
- Valid driver’s license and reliable transportation required
- Must maintain appropriate auto insurance coverage
Specific Duties
The duties listed below are primary functions of the job, and are not intended to be all-inclusive.
- Develop a program work plan in conjunction with the Westbay work plan
- Define roles and responsibilities of program staff
- Train and supervise staff
- Conduct performance appraisals of program staff
- Oversee day to day operation of the case management programs
- Monitor programs for compliance with federal, state, and local guidelines
- Monitor client intake and certification eligibility to ensure compliance with established guidelines
- Ensure all client referrals are assigned to staff and monitored
- Monitor each caseload
- Maintain accurate program, statistical, and client records and submit reports as required
- Assist with the preparation of records for billing purposes
- Supervise and train interns, monitoring their work
- Provide case management services, including initial contact with the client.
- Conduct a client assessment and development of a care plan
- Provide follow-up services to senior clients in Home Care programs and those designated by OHA as abuse or protective clients
- Conduct home visits as needed
- Engage family and community support systems as available
- Provide supportive counseling and advocacy to clients and their families
- Coordinate services with other community resources
- Determine eligibility needed resources and aid in the application process
- Monitor clients and modify care plans as needed
- Inform clients of their rights and responsibilities
- Support client’s efforts to have their voice heard and to get needed services.
- Offer advocacy on the client’s or family’s behalf when needed services are not being adequately provided by an organization within the community’s service delivery system
- Recognize potential conflicts of interest and refer consumers to the next appropriate service to ensure advocacy needs are met
- Provide information and referral services to telephone callers and clients of Westbay Community Action
- Maintain information needed for data collection and enter into the agency's data system
- Participate in meetings and trainings as needed and in public policy forums when appropriate
- Educate the general community about senior services and Westbay
