Position Title: Direct Care Worker (DCW)
Reports To: Director of Nursing
Job Summary
At Grand Haven, the Direct Care Worker provides compassionate support and assistance to our tenants. The DCW helps clients maintain independence and improve quality of life by assisting with daily living activities.
Looking for: Part-time 1st, 2nd & 3rd shift!
Essential Duties and Responsibilities
- Assist tenants with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting.
- Administer or assist with medications as directed and document accordingly.
- Monitor and document tenant's health status.
- Implement individualized care plans and support goals.
- Respond appropriately to emergencies and follow safety procedures.
- Maintain confidentiality and comply with all agency policies and regulations.
- Communicate effectively with clients, family members, supervisors, and healthcare professionals.
