Per Diem Physical Therapist role providing 1-on-1 care within a senior living community (Independent Living Facility) in Marlborough, MA with minimal travel and no productivity quotas.
$55/hour (per-visit compensation in an on-site outpatient setting)
All patients are treated on-site

This is not traditional home health. All care is delivered within one community, allowing for an efficient schedule without excessive driving. You will provide outpatient-style care without the pressures of a high-volume clinic environment.
What You’ll Do
- Provide 1-on-1 outpatient treatment sessions in an on-site senior living community
- Complete evaluations and establish plans of care
- Reassess and update treatment plans as needed
- Communicate with patients, families, and the care team
- Complete documentation using a streamlined system
- Participate in supporting the integration of therapy services within the community through consistent clinical presence
This role begins with an existing patient base and is expected to grow as additional residents are referred within the community. You will help establish a strong therapy presence within the community by delivering excellent clinical care and building positive relationships with residents and staff.

- This is a per diem role with a flexible schedule. Weekly visits will vary based on patient referrals, with opportunities to increase as the community continues to grow.
Qualifications
- Active Physical Therapist license in Massachusetts
- Ability to work independently in a community-based outpatient setting
- Experience in outpatient or geriatric populations preferred
- Strong clinical judgment and communication skills
- Interest in helping grow outpatient therapy services within a senior living community.
