Position Overview
The Full-Time LVN Field Nurse provides skilled nursing care to primarily geriatric patients in their homes while supporting the overall coordination and management of patient care.
The LVN works collaboratively with the RN case manager, physicians, patients, families, caregivers, and other members of the interdisciplinary home health team to promote patient safety, improve outcomes, and help patients remain safely at home.
Essential Responsibilities
- Provide skilled nursing services in patients' homes in accordance with the plan of care and physician orders
- Perform patient assessments and monitor changes in condition
- Administer medications and treatments as ordered
- Provide patient and caregiver education related to disease processes, medications, treatments, and safety
- Communicate significant changes in patient condition to the clinical coordinator and physician as appropriate
- Collaborate with the home health team to promote continuity and quality of care
- Support RN case managers with patient care coordination and follow-up as appropriate
- Complete accurate and timely clinical documentation
- Participate in care planning and contribute to the development of individualized patient goals
- Help identify changes in patient condition and potential risks for hospitalization
- Maintain compliance with agency policies, Medicare requirements, and applicable regulations
- Provide care in accordance with professional standards and the patient's individualized plan of care
Schedule
- Full Time Per Visit position
- Primarily weekday field visits
- Occasional weekend visits may be required based on patient needs and agency scheduling
Qualifications
- Current, unrestricted Texas LVN license
- Current valid driver's license
- Reliable transportation and the ability to travel to patients' homes throughout the agency's service area
- Minimum of one year of LVN experience in a clinical setting preferred
- Home health, hospice, geriatrics, or other community-based nursing experience preferred
- Strong assessment, communication, organization, and time-management skills
- Ability to work independently while functioning effectively as part of a collaborative team
- Professional, dependable, compassionate, and patient-centered approach to care
Physical & Environmental Requirements
This position requires the ability to:
- Travel to and provide care in patients' private residences
- Frequently stand, walk, bend, stoop, kneel, and reach
- Assist with patient mobility and transfers as appropriate to the patient's plan of care and safety needs
- Lift and carry necessary clinical supplies and equipment
- Work in a variety of home environments
