Purpose of Position
The Registered Nurse (RN) Navigator provides direct nursing care and comprehensive case management services to pregnant women, infants, and families enrolled in Healthy Start services. Working within a team-based care model, the RN partners with the Care Coordinator, medical providers, and community partners to ensure high-quality, coordinated care.
The RN Navigator implements, monitors, and evaluates individualized care plans through home visiting, ensuring client safety, promoting positive maternal and infant health outcomes, and addressing identified risks. The position requires strong clinical judgment, independent decision-making, and timely communication with the healthcare team.
Responsibilities include but not limited to the following:
· Provide direct nursing care in accordance with established policies and protocols.
· Conduct home visits including starts of care, resumptions, recertifications, and routine visits.
· Perform medication reconciliation at each visit.
· Conducts comprehensive head-to-toe nursing assessments in addition to assessing safety risks, abuse, and neglect.
· Coordinate care across providers and disciplines.
· Develop and update care plans with clients and families.
· Communicate changes in patient condition to physicians.
· Maintain accurate, timely documentation compliant with program requirements.
· Provide patient education and support.
· Participate in team meetings and quality improvement activities.
· Other duties as assigned.
Qualifications, Training and Experience
· Graduate of an accredited School of Professional Nursing
· Active Registered Nurse (RN) license
· Basic Life Support (BLS) – American Heart Association (AHA)
· Valid driver’s license with proof of auto insurance
· 2+ years of prenatal nursing experience in an acute care setting
· 1+ year of RN experience in an acute medical-surgical environment
· Postpartum and/or obstetrics (OB) experience
Ability To:
· Ability to perform duties requiring
o Full range of motion, manual dexterity, and hand-eye coordination
o Prolonged standing and walking
o Lifting 20–50 pounds occasionally, 10–25 pounds frequently
o Normal (corrected) hearing and vision
o Ability to ambulate independently, including climbing stairs
o Ability to push, pull, and assist with patient movement
· Must be able to read, write, and effectively communicate with patients
· Requires travel to patient homes
· Always required adherence to standard precautions and use of personal protective equipment (PPE)
Pay: $65,000.00 - $87,000.00 per year
