Responsibilities
- Dispense prescription medications accurately while ensuring compliance with all regulatory standards and safety protocols.
- Provide expert consultation to healthcare providers regarding medication selection, dosing, interactions, and side effects.
- Conduct comprehensive patient assessments to determine appropriate medication therapy, including evaluating medical histories and current health status.
- Manage pharmacy inventory efficiently, including ordering, stock rotation, and compounding medications as needed using aseptic techniques.
- Administer immunizations and perform medication administration procedures in accordance with medical management plans.
- Utilize pharmacy software and electronic health record (EHR) systems to document patient interactions, prescriptions, and medication histories accurately.
- Support patient education initiatives on medication use, potential side effects, and adherence strategies to promote optimal health outcomes.
Qualifications
- Valid pharmacist license with current registration in the relevant jurisdiction.
- Prior experience in LTC (Long-Term Care) pharmacy or critical care settings is highly desirable.
- Completion of a pharmacy residency program or equivalent clinical pharmacy experience.
- Demonstrated proficiency with pharmacy software systems and EMR/EHR platforms.
- Strong knowledge of physiology, anatomy, medical terminology, and pediatric pharmacology.
- Experience with compounding medications and IV infusion procedures, including aseptic technique and IV insertion skills.
- Familiarity with hospital or acute care environments, pain management protocols, and immunization administration.
- Excellent patient service skills with the ability to perform patient assessments effectively.
- Knowledge of HIPAA regulations to ensure patient confidentiality and data security.
