Key Responsibilities:
- Provide in-home patient care, including assessments, medication management, and patient education.
- Collaborate with physicians and other healthcare professionals to create personalized care plans.
- Ensure high-quality, compassionate care tailored to each patient’s needs.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and families on healthcare management and safety.
- Maintain accurate, up-to-date patient records and documentation.
Qualifications:
- Current, active RN license in [State].
- years of experience in home health or similar settings (preferred but not required).
- Strong clinical skills and ability to assess and respond to patient needs.
- Excellent communication skills and ability to work independently.
