Summary
The Case Manager conduct telephonic visits using comprehensive assessment tools for members enrolled in Managed Long‑Term Services and Supports (MLTSS). Perform in‑person assessments for non‑MLTSS members to determine medical needs and support appropriate service referrals (e.g., adult medical daycare, pediatric medical daycare, personal care assistant services, nursing facility custodial care, Personal Preference Program, and MLTSS enrollment).
Requirements
- An active and unrestricted RN license for the state of New York
- Associate's degree in Nursing. Bachelor's degree is preferred
- 3 years of nursing experience underneath license
- 3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care
- 2 years Healthcare and/or managed care industry experience
- 2 years of Case Management experience
- Experience providing care management for Medicare and/or Medicaid members
- Experience working with individuals with SDoH needs, chronic medical conditions, and/or behavioral health
- Bilingual preferred: Fluent in English & Spanish
