Position Summary
The Resident Care Coordinator is a key leadership position responsible for ensuring exceptional resident care, coordinating communication among families, healthcare providers, and caregivers, and supporting the day-to-day clinical and operational needs of the community.
This individual serves as the bridge between residents, families, caregivers, and management while helping maintain the highest standards of care, compliance, and customer service. The Resident Care Coordinator will work closely with the leadership team to ensure residents receive compassionate, high-quality care while supporting the daily operations of the community.
This role is ideal for someone who is compassionate, highly organized, enjoys leading others, and is looking to grow into a larger leadership role within senior living.
Key Responsibilities & Resident Care
- Coordinate resident care plans and ensure services are delivered according to each resident’s individualized needs.
- Conduct and assist with new resident assessments, admissions, and move-ins.
- Monitor residents for changes in condition and communicate updates to families, physicians, and healthcare providers.
- Ensure resident documentation is accurate, timely, and compliant with state regulations.
- Provide assistance with medication passes when needed in accordance with Florida regulations, community policies, and training requirements.
- Help resolve resident and family concerns with professionalism, empathy, and urgency.
Caregiver Leadership
- Train, mentor, and support caregivers to ensure quality care and consistency.
- Assist with caregiver scheduling and staffing coordination.
- Provide coaching, accountability, and ongoing support to direct care staff.
- Promote teamwork, professionalism, and a resident-first culture throughout the community.
Family & Healthcare Coordination
- Serve as a primary point of contact for resident families regarding care updates and concerns.
- Coordinate communication with physicians, pharmacies, hospice agencies, home health providers, therapists, and other healthcare professionals.
- Ensure timely follow-up on physician orders, appointments, care recommendations, and resident needs.
Operations Support
- Assist with the day-to-day operations of the community to ensure excellent resident care and customer service.
- Support compliance with AHCA regulations, community policies, and quality assurance initiatives.
- Maintain confidentiality and ensure resident rights are always protected.
- Step in wherever needed to support residents, caregivers, and the management team.
- Participate in community meetings, training, and ongoing process improvements.
Qualifications
- Experience in Assisted Living, Memory Care, Skilled Nursing, or a related healthcare setting preferred.
- Experience supervising, training, or mentoring caregivers strongly preferred.
- Medication Technician certification or willingness to obtain required certifications.
- Strong organizational, communication, and interpersonal skills.
- Ability to multitask, prioritize, and remain calm in a fast-paced environment.
- Compassionate, dependable, professional, and solution-oriented.
- Basic computer proficiency and experience with electronic documentation preferred.
Preferred Qualifications
- Experience conducting resident assessments.
- Familiarity with Florida Assisted Living regulations and AHCA compliance.
- Previous leadership experience in senior living or healthcare.
