We are looking for a nurse manager that will be part time and responsible for supervising care managers as well as onboarding new patients into Enabled Health and managing complex illness with oversight by a nurse practitioner. Part of your time will be on the road visiting facilities (30-50%) and part of your time will be remote. We are looking for someone with high integrity who is able to learn quickly and who has deep compassion for patients.

Join our dynamic healthcare team as a Part Time Nurse Care Manager Supervisor, where you will lead and oversee patient care coordination within a fast-paced, multidisciplinary environment. This role is designed for experienced nursing professionals who are passionate about delivering exceptional patient care and improving health outcomes. As a supervisor, you will guide a team of care managers, ensure compliance with healthcare standards, and foster a collaborative approach to case management across various clinical settings. Your expertise in hospital, trauma, and behavioral health environments will be instrumental in shaping comprehensive care plans that meet the diverse needs of our patient populations.
Duties
- Supervise and mentor a team of care managers, ensuring high-quality patient assessments and care coordination.
- Conduct regular case reviews, utilization reviews, and data collection to optimize patient care strategies.
- Collaborate with social workers, behavioral health specialists, and physicians to develop individualized care plans that address physical, mental health, and behavioral needs.
- Lead efforts in patient assessment, physiology and anatomy knowledge application, and behavioral therapy integration to enhance treatment outcomes.
- Ensure adherence to hospital policies related to HIPAA regulations, Medicare guidelines, and managed care protocols.
- Facilitate communication between care managers and medical providers.
- Promote leadership within the team by fostering professional development and supervising documentation review processes for accuracy and completeness.
Experience
- Proven experience working in dynamic healthcare environments.
- Experience working with seniors and managing chronic health conditions
- Extensive background in nursing with specialized experience hospital medicine, primary care, or cardiology.
- Prior supervisory or management experience supervising nursing teams or case management staff.
- Familiarity with home health services, hospice care, behavioral health counseling, addiction counseling or mental health therapy is highly desirable.
- Knowledge of medical terminology, anatomy, physiology, discharge planning procedures, and medical records management.
- Experience in data collection, utilization review processes, and adherence to NCQA standards for quality assurance.
- Strong understanding of social work principles combined with leadership skills to guide multidisciplinary teams effectively.
- Ability to navigate complex healthcare regulations including HIPAA compliance and Medicare requirements while maintaining patient-centered focus.
