The Licensed Vocational Nurse (LVN) ) is responsible for providing direct health services to residents in a safe and supportive environment, as well as health system navigation services, at Ted’s Place (Whittier), Pathway Home (Santa Fe Springs) and Hondo Center (Downey). This role requires an even distribution of work time between the three sites, with site assignments subject to change based on program needs.
The LVN acts as a patient advocate, conducts limited physical assessments, supports medication management, and collaborates closely with primary healthcare providers. In addition, this position plays a key leadership role in agency-wide clinical coordination and health compliance efforts.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Program Responsibilities
- Maintain a calm, friendly and welcoming atmosphere within First Day’s facility promoting a clean, orderly and healthy environment at all times;
- Provides basic nurse assistant services including: monitoring of medications, monitoring vitals, direct communication with participant’s medical providers for referrals to other providers and community health supports, health education and instruction to patients, and instruction and in-servicing to counseling line staff including monitoring of self-administration of prescribed medication;
- In collaboration with Program Directors and clinical staff, designs and implement a comprehensive individual health services plan for each participant that includes a broad range of medical services including providing support to clients with physical and emotional need management and referrals to other outside service providers;
- Maintain individual health records and documentation participants’ current medication(s), including: name(s) of medical care provider prescribing medication, name(s) of medication, dosage(s), frequency, duration, start and end dates, and any allergy information;
- Provide report of any incidents occurring during daily shift in writing. If incidents meet HSH reporting criteria, ensure Incident Report is submitted to HSH within 12 hours of incident;
- Serve as liaison between Program Staff and Medical Providers, in order to coordinate best care of clients
- Maintain electronic health records and hard copy documentation of all services provided;
- Assist with conflict resolution by modeling and encouraging positive methods of problem-solving, respectful communication and self-restraint;
- Maintain and execute confidential information according to HIPAA standards;
- Participate in weekly case conferencing, attend staff trainings and department meetings as needed to ensure optimal care for participants;
- Log, file and enter source documents and related data into client files, HMIS, CHAMP, and other data capture systems;
Additional Leadership Responsibilities
- Oversee and lead agency-wide medical supply ordering, ensuring consistent and adequate inventory across all programs.
- Manage and maintain the relationship with PIH Health, ensuring visiting doctors receive all necessary support and documentation.
- Maintain CPR trainer certification and provide CPR trainings agency-wide to ensure staff remain current with required life-saving skills.
EDUCATION/EXPERIENCE/TRAINING
Required:
- Current and valid California Licensed Vocational Nurse certification, with one (1) year work experience as an LVN
- CPR/First Aid Training Certification
- Ability to administer tuberculosis (TB) skin test and providing results
- Proficiency in Microsoft Office and Web-based programs, HMIS, Electronic Health Records system and any other database software(s) used to track work outputs;
- Possess a thorough understanding of the importance of confidentiality and non-disclosure to the general standards set forth by HIPAA
- Valid CA Driver’s License, auto insurance and reliable transportation. If providing driving support on behalf of First Day, must be able to meet vehicle insurance requirements to be covered by First Day’s Vehicle Insurance policy at all times while employed.
Preferred:
- Two years of experience in social services, hospitality, or health care including experience dealing with substance use in clients and crisis intervention
- CHAMP and/or HMIS Training Certification
- Minimum two years of non-profit experience with the homeless population and/or case management in a supervisorial or management role.
