Registered Nurse Case Manager
Responsibilities:
- Provide comprehensive case management services to patients in various healthcare settings, critical care, behavioral health, and ER.
- Conduct patient assessments and develop individualized care plans.
- Coordinate and collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and their families on disease management, treatment options, and available resources.
- Advocate for patients' rights and ensure their needs are met.
- Document all patient interactions and interventions accurately and timely.
Requirements:
