What You'll Do
- Provide direct patient care within the RN scope of practice in a primary care clinic
- Triage incoming patient calls and walk-ins, offering clinical advice as appropriate
- Review labs, imaging, and diagnostic results; collaborate with providers on abnormal findings
- Develop individualized care plans and coordinate care with Enhanced Care Management
- Coordinate referrals to specialty care providers
- Deliver patient education on chronic disease management, wellness, and prevention
- Partner with an integrated care team — physicians, behavioral health, and support staff
- Support quality improvement initiatives and maintain accurate EHR documentation
- Perform monthly emergency cart and oxygen tank readiness checks
- Provide culturally respectful care to a diverse patient population
What You'll Bring
- Bachelor of Science in Nursing (BSN) — required
- Active California RN license — required
- 2+ years of nursing experience, ideally in primary care, outpatient, ambulatory care, or community health
- Current BLS certification — required (ACLS and PALS preferred)
- Proficiency with Electronic Health Records (EHR) and Microsoft Office
- Strong triage, health assessment, and care coordination skills
- Excellent communication with patients, families, and interdisciplinary teams
- Bilingual skills a plus — Spanish, Mandarin, or Vietnamese especially valued (not required)
Pay: $55.00 - $64.00 per hour
