Background
Union Health Service is a multidisciplinary outpatient clinic with integrated behavioral health, and specialty physician support. UHS provides pharmacy, laboratory, radiology, ultrasound, optometry, & nutritional services for patients.
Position Summary
The Primary Care Physician “PCP” provides comprehensive, patient-centered primary care services within a multidisciplinary outpatient setting. This role includes preventive care, acute and chronic disease management, and care coordination across the healthcare continuum.
· Family Medicine physicians will manage patients across the lifespan
· Internal Medicine physicians will manage adult patients.
Clinical Responsibilities
· Provide comprehensive primary care, including preventive services, acute care, and chronic disease management.
· Perform age-appropriate history and physical examinations for new and established patients.
· Diagnose and manage a wide range of medical conditions in accordance with evidence-based guidelines.
· Deliver urgent care services for same-day or walk-in needs, as applicable.
· Order and interpret diagnostic studies, including laboratory tests and imaging.
· Develop and implement individualized treatment plans.
· Provide health maintenance counseling, including disease prevention and lifestyle modification.
· Manage chronic conditions (e.g., diabetes, hypertension, COPD, obesity) with a focus on outcomes and patient engagement.
· Coordinate care with specialists, behavioral health, and ancillary services.
· For Family Medicine physicians: provide pediatric and adult care consistent with training and scope.
Panel Management
· Serve as the primary provider for an assigned patient panel, ensuring continuity and longitudinal care.
· Collaborate with nursing staff, medical assistants, pharmacy, and care management teams to deliver team-based care.
· Participate in care coordination activities, including transitions of care
Administrative Responsibilities
· Review and act on laboratory, imaging, testing, and consultation reports in a timely manner.
· Ensure appropriate follow-up for abnormal findings and closed-loop communication with patients.
· Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
· Participate in after-hours and weekend call coverage as assigned.
· Attend provider, staff, and committee meetings.
· Contribute to the development and implementation of clinical protocols, workflows, and quality improvement initiatives.
· Maintain compliance with organizational policies, regulatory standards, and clinical best practices.
Expectations
· Meet established clinical quality, access, and patient experience metrics.
· Engage in continuous professional development through CME and required certifications.
· Stay current with evolving clinical guidelines and standards of care.
· Demonstrates proficiency in electronic health systems (EHR) systems, including timely documentation, order entry, inbox management, and results review.
· Work Saturdays as scheduled
Qualifications
· Completion of an undergraduate degree.
· MD or DO degree from an accredited medical school.
· Completion of an accredited 3-year Internal Medicine or Family Medicine residency program.
· Board-certified or board-eligible in Internal Medicine or Family Medicine.
· Board-certification must remain current while practicing at UHS
· Experience in an outpatient medical practice is preferred
Licensure & Certifications
· Active and unrestricted Illinois Physician License.
· Current DEA registration and NPI.
· Current Medicare license & PECOS certification
· Current BLS certification.
Pay: From $117.82 per hour
