We are looking for a dedicated Registered Nurse (RN) to join our home health team on a full-time basis. This position involves providing skilled nursing care to patients in their homes and supporting their recovery and ongoing health needs.
Provide skilled nursing care in patients’ homes, including wound care, medication administration, IV therapy, catheter care, and other treatments as ordered
Assess patient conditions and report changes to the physician and care team
Develop and follow individualized care plans
Educate patients and families on care needs and treatment plans
Complete timely and accurate documentation
Coordinate care with other healthcare professionals
Participate in on-call rotation as needed
Active Arizona RN license
At least 1 year of nursing experience preferred
Valid driver’s license, reliable transportation, and auto insurance
Ability to work independently in the field
Full-time
Monday through Friday
On-call rotation as needed
Occasional weekend availability
Field-based – travel to patient homes within the assigned service area
