The Coordinator is responsible for coordinating patient care activities for patients in need of enhanced care coordination. The Coordinator will ensure patients receive timely and high quality healthcare and support services through the use of care protocols, care plans, self-management goals and referrals to the most appropriate resources.
1. Collaborate with other members of care team to create and implement plan of care for patients. (10%)
2. Work with patients both in person and over the phone to remind and review their plan of care and progress towards their goals. (20%)
3. Facilitate referrals and ensure patients receive appropriate and timely screenings and tests. (30%)
4. Provide relevant self-management support for patients as identified by care team. (10%)
5. Ensure timely documentation within electronic health record and assist with data collection for specific clinical quality measures. (20%)
6. Perform other duties as assigned. (10%)

Employee is required to sit continuously and walk frequently. Employee must lift/carry a light (1-20 lbs.) medium (21-50 lbs.) and heavy (50 lbs. +) load occasionally. Employee may be required to lift over 100 lbs. with assistance. Employee is frequently required to bend, squat, crawl, kneel, climb, lift waist to overhead. Employee is occasionally required to reach forward and reach overhead. Employee may be required to do simple grasping and pushing/pulling.

Associates degree or healthcare credential (LPN, RN, etc) required. Bachelor’s preferred.
