Position Summary
Prime Home Health is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to provide skilled nursing care to a patient in the Antelope Valley area in the comfort of the patient's home.
This position offers a consistent weekday schedule and the opportunity to build meaningful relationships with patients and families while gaining valuable hands-on experience in home health.
Schedule
- Monday through Friday
- 7:00 AM to 11:00 AM
- 4 hours per day
- Antelope Valley area
Responsibilities
- Provide skilled nursing care in the home setting
- Follow individualized care plans and physician orders
- Monitor, document, and report patient progress and changes in condition
- Administer medications and treatments as prescribed
- Collaborate with the interdisciplinary healthcare team
- Educate patients and families regarding care plans and disease management
- Maintain accurate and timely clinical documentation
Qualifications
- Current and active California LVN license
- Current BLS certification
- Current TB test or chest X-ray (if applicable)
- Valid driver's license and reliable transportation
- Strong communication and documentation skills
- Compassionate, dependable, and professional demeanor
- Experience with G-tube (GT), tracheostomy (Trach), and ventilator (Vent) patients is strongly encouraged
