Golden Castle Adult Day Health Center ( http://www.goldencastlecenter.org/ ) is a licensed community-based adult services program (CBAS) that provides services to older persons and adults with chronic medical, cognitive, or mental health conditions and/or disabilities in a coordinated program of professional and compassionate services in a community-based group setting.
Full-Time or Part-Time, Monday through Friday, no evening hours.
DUTIES AND RESPONSIBILITIES
- Provide a comprehensive nursing assessment of participants upon admission
- Development and implementation of person-centered nursing care plans and participant assessments using evidence-based nursing practices and interventions
- Participate in discharge planning from hospitals and/or Skilled Nursing Facilities
- Collect data and provide focused assessment on a quarterly basis.
- Identifies participant care requirements by establishing personal rapport with participants
- Establishes a compassionate environment by providing emotional, psychological, and spiritual support to participants, friends, and families
- Promotes participant independences by establishing participant care goals; teaching participant, friends and family to understand condition, medication, and self-care skills; answering questions
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against participant care goals and standards, making or recommending necessary adjustments
- Builds collaborative relationships and regular communication channels with each participant's PCP and the participant's circle of community-based services.
- Maintains safe and clean working environment by complying with procedures, rules, and regulations

MINIMUM REQUIREMENTS
License/Certification:
- Current California RN license with no history of sanctions
Experience:
- Case management, or community nursing experience working with individuals with chronic health, behavioral health and social challenges preferred
- Geriatric experience preferred
Pay: $40.77 - $49.10 per hour
