We are seeking an experienced, thoughtful Hospice Social Worker who understands the importance of presence, communication and practical support at the end of life. You will work closely with patients, families and an interdisciplinary team that respects the social worker’s role in delivering truly comprehensive hospice care.
Candidates with a BSW, MSW, LSW or LCSW will be considered. Responsibilities, compensation and scope will be aligned with education, licensure and hospice experience.
What You Will Do
- Complete psychosocial assessments within your education and scope of practice
- Provide emotional, psychosocial and practical support to patients and families
- Assist with advance care planning, goals-of-care discussions and end-of-life concerns
- Help families navigate caregiving challenges, grief, changing family dynamics and community resources
- Coordinate supportive services and connect families with appropriate programs
- Collaborate with nurses, physicians, chaplains, CNAs and other interdisciplinary team members
- Participate actively in interdisciplinary group meetings and care planning
- Maintain accurate, timely and compliant documentation
- Travel to patient homes and facilities throughout your assigned service area

Qualifications
- BSW or MSW from an accredited social work program
- Active Illinois license when required for the position and assigned scope
- Hospice, palliative care, home health or healthcare social work experience strongly preferred
- Understanding of end-of-life care, family systems, grief and community resources
- Strong assessment, communication, organization and documentation skills
- Ability to work independently while remaining connected to the interdisciplinary team
- Reliable transportation and willingness to travel throughout the assigned territory
- Commitment to dignity-focused, patient- and family-centered care
Physical and Environmental Requirements
This position requires comfort working in real homes and healthcare facilities where layouts vary, pets may supervise and not every doorway was designed with healthcare in mind. Team members must be able to safely navigate these environments and perform the essential responsibilities of the role.
Compensation
Compensation is based on education, licensure, hospice experience and assigned scope:
- BSW / LSW: $45,000–$65,000 per year
- MSW: $55,000–$80,000 per year
- LCSW: $75,000–$105,000 per year
Part-time compensation will be structured according to the agreed-upon schedule and role responsibilities.
