Duties
- Provide direct patient care with a focus on assessment, medication administration, wound care, and vital signs monitoring.
- Supervise and support nursing staff, ensuring adherence to clinical protocols and quality standards.
- Coordinate interdisciplinary communication for effective discharge planning and case management.
- Oversee medication management including IV infusion, injections, and tube feeding procedures while maintaining strict compliance with HIPAA and confidentiality policies.
- Conduct comprehensive patient assessments, including physical examinations and diagnostic evaluations such as laboratory specimen collection and processing.
- Manage electronic health records (PCC systems) documentation accurately using platforms like Epic or Cerner.
- Ensure compliance with long-term care regulations and safety protocols specific to skilled nursing facilities or assisted living environments.
Qualifications
- Valid LVN license with current state licensure required.
- Demonstrated expertise working with geriatric patients, individuals with developmental disabilities or memory impairments like Alzheimer’s disease.
- Strong knowledge of medical terminology, physiology, pharmacology related to pain management and infection control practices.
- Excellent communication skills with the ability to lead a team effectively while maintaining clinical confidentiality policies.
- Experience working within long-term care regulations or sub-acute care settings; knowledge of hospice or home & community care is a plus.
