JOB DEFINITION:
Bringing education, training, and direct concierge-style services for needed mental health and substance use services to three Long Island JCCs; Mid-Island JCC, MarionGural JCC, and Sid Jacobson JCC.
This position is grant funded by The UJA.
EDUCATIONAL REQUIREMENTS:
Master's Degree in Social Work or Mental Health Counseling; licensed preferred.
EXPERIENCE REQUIRED:
3 years of experience:

Working with adults and youth diagnosed with mental health and/or substance use disorders

Making referrals to community resources throughout Long Island
-
Providing culturally-attuned support to the target population
Valid NY driver's license with safe driving record required.
DUTIES AND RESPONSIBILITIES:
- Conducts intake assessments to identify behavioral health, social, and logistical needs of participants.

Works closely with participants to match them with culturally competent providers and resources, facilitate appointments, and provide follow-up to ensure engagement and continuity of care.
- Provides individual services and support virtually, in the JCC offices, or in community locations based on participant need and preference.

Provides group services and support virtually, in the JCC offices, or in community locations based on participant need and preference.

Schedules and/or facilitates trainings on topics like Mental Health First Aid and opioid overdose prevention for JCC members and staff.

Participates in tabling events and educational outreach to build awareness of behavioral health topics and agency support.

Collects, analyzes, and incorporates participant and partner feedback to refine workflows and protocols.

Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress

Complete QI Training during onboarding and participate in QI refresher training as needed.
